Mapping Mental Health Support in High Peak

The High Peak Health and Wellbeing Partnership’s Mental Health and Wellbeing Sub Group is seeking to map the mental health support on offer to residents of High Peak.  

They have created a survey to capture as much of our local offer as possible and would really welcome your support to complete within the next fortnight.

The intention is to use the information gathered to produce two promotional leaflets to be delivered to High Peak households – one focusing on support for adults and another on support for children and young people. They aim to produce and distribute these in early June. So timescales for gathering the information are tight.

Complete the survey by midday on Monday 17 May – remember to complete one form per service/project/group.

Share the survey widely with anyone and everyone in your networks that you think may be able to contribute.
